Referrals to affiliated community physicians/providers in Oregon and Washington

Affiliated community physicians/providers are not part of the Northwest Permanente Medical Group (NWPMG). They are physicians/providers credentialed by Kaiser Foundation Health Plan of the Northwest and/or who have a contract with the NWPMG.

If your NWPMG physician decides that you require covered services not available internally from an NWPMG physician, he or she may refer you to an affiliated community physician/provider. You must have a written, authorized referral from Kaiser Permanente in order to receive a covered service from an affiliated community physician/provider.

Added Choice and PPO Plus plans

As an Added Choice or PPO Plus member, you have access to all that Kaiser Permanente offers, plus the option to seek care from providers outside of our network.

Senior Advantage plans

If you are a Medicare member, most providers who are listed on the Kaiser Permanente Provider Directory are available to you as a Kaiser Permanente member. Please note that specialists may require a referral from your Primary Care Physician or other Kaiser Permanente Provider.
