Network Access Disclosures

Important information for members looking for the following:

 

Oral and maxillofacial surgery services in Benton County for the following networks:

  • PPO Plus 
  • PPO Plus + Dental Choice

 

Kaiser Permanente Northwest currently does not have in-network oral and maxillofacial surgery providers within its 60-mile travel standard for all members residing in Benton county. Check your evidence of coverage (EOC) to see which oral and maxillofacial surgery services can be covered under your medical benefits. Kaiser Permanente Northwest will determine whether the care or treatment required is within the category of oral surgery or dental services.

If these services are not available from an in-network provider, you may be able to receive benefits for services provided by an out-of-network provider at applicable in-network deductible and cost shares (copays or coinsurance).

Behavioral health emergency services in Clark, Skamania, Klickitat, Benton, Columbia, Franklin, Walla Walla, Whitman, Yakima, Kittitas, Garfield, Asotin, King, Island, Skagit, Snohomish, Whatcom, San Juan, Pierce, Spokane, Adams, Lincoln, Ferry, Stevens, Pend Oreille, Mason, Thurston, Okanogan, Chelan, Douglas, Grant, Cowlitz, Lewis, Wahkiakum, Pacific, Greys Harbor, Clallam, Jefferson, & Kitsap counties for the following networks:

  • PPO Plus 
  • PPO Plus + Dental Choice 

Behavioral health emergency services in Clark and Cowlitz counties for the following networks:

  • Classic Complete
  • Classic + Dental Choice 

 

Kaiser Permanente Northwest currently does not have in-network mobile rapid response crisis team services providers in the counties outlined above. There may also be a limited number of crisis stabilization units, evaluation and treatment centers, and medically managed or medically monitored withdrawal management (detox) facilities within the state’s 30-minute travel standard for some members residing in these counties. However, providers of behavioral health emergency services are considered in-network when accessed through calls to 911 or 988 or other crisis triage sources, and therefore there is no need to contact Customer Service to obtain authorization for these services.
