How to Qualify and Apply for QUEST Integration in Hawaii

Have you already applied for and been approved for Medicaid?

Kaiser Permanente participates in QUEST Integration for those who live on Oahu or Maui. Once you qualify for QUEST (Hawaii Medicaid) through the State of Hawaii, Department of Human Services, Med-QUEST Division, you’ll need to apply for health coverage. When applying, be sure to choose Kaiser Permanente.

3. Submit your QUEST Integration application.

Go to www.mybenefits.hawaii.gov and print out a QUEST Integration application. Send completed and signed applications to your local Med-QUEST Division eligibility office

For information about applying and enrolling in QUEST Integration, please call Med-QUEST at 1-877-628-5076.

In addition to your completed QUEST Integration application, you may be asked to provide other documents, such as: 

  • Identification
  • Proof of U.S. Citizenship or Immigration status
  • Proof of residency (you must live in the state in which you apply)
  • Household income

4. Receive approval for QUEST Integration.

It could take the state up to 45 days to see if you are eligible for QUEST Integration coverage.

Once your paperwork has been reviewed and approved, you’ll receive a welcome packet from the state.

After you’ve been approved for QUEST Integration, here’s what you need to do to select QUEST Integration with Kaiser Permanente or another health plan:

1. Choose your health plan.

Once approved as Medicaid eligible, you will automatically be assigned to a participating health plan (which may or may not be Kaiser Permanente).

  • MQD will initially assign family members to the same plan.
  • You then have the option to stay with the assigned plan or choose a different health plan.

 

You will receive a packet from Med-QUEST, which will include a choice form.

  • If you choose to stay with the assigned health plan, there’s nothing more to do.
  • If you would like to change plans, you should make a selection using the choice form and return it to MQD within 15 days of receiving the packet. Plan selection can also be made by calling MQD Customer Service at 1-800-316-8005.
  • Family members are allowed to choose different health plans.
  • The new health plan will start on the first day of the following month after the selection was made. Until then, you will stay with the plan assigned.

You can change plans one time within 90 days from initial enrollment. Once the 90-day grace period ends, changes can only be made during annual open enrollment (usually October 1 – 31, to be effective January 1 of the following year).

 

Upon enrollment to Kaiser Permanente, we will send you a welcome packet and ID card.

An immediate family member is a family member who lives in the home of a current Kaiser Permanente member and is one of the following:

  • Spouse (including Domestic Partners)
  • An unmarried dependent child under age 21
  • A disabled dependent over the age of 21
  • Married/Unmarried/Stepparents of children under age 21
  • Foster child or stepchild
  • Legal guardian
  • A grandparent, parent, guardian or other relative who applied for QUEST Integration on behalf of a current Kaiser Permanente member that is under 21.

2. Receive your Kaiser Permanente ID card(s) once you’re enrolled.

Please be sure to bring your ID card(s) any time you receive services.
