About this program

Act to have your wishes honored by making them known. Life Care Planning begins with choosing someone (called a health care agent/decision maker) to speak for you if you’re unable to speak for yourself. This free class will help you: choose your health care agent, share your wishes for future health care decisions, and complete your Advance Directive. Registration instructions at bottom of the page.
