About this program

Act to have your wishes honored by making them known. Life Care Planning begins with choosing someone (called a health care agent/decision maker) to speak for you if you’re unable to speak for yourself. This free online class will help you: choose your health care agent, share your wishes for future health care decisions, and complete your Advance Directive. You can join this virtual class from your computer and/or phone. Join us, along with your chosen decision maker by registering today. Kaiser Permanente members can register for this program online in our appointment center or by clicking the blue "Register now" button above. Note: If you register more than 4 days in advance, you will be mailed class materials.
