Am I eligible to enroll?

You may be eligible to enroll Kaiser Permanente Dual Complete Plan if you have both Medicare and Medicaid and live in Hawaii (Oahu and Maui only).

You can enroll during the annual enrollment period from October 15 through December 7, or any other time if you have a qualifying life event such as being new to the area, recently lost coverage, or have had a change in your dual-eligible or LIS (Low Income Subsidy) status.

You can enroll online in 4 simple steps:

Go to our Medicare homepage.
Enter your ZIP code.
Select “Yes” under the “Medicaid?” dropdown and click “Explore plans.” (If you’re visiting the website between October 1 and December 31, you’ll need to select the coverage year.”
Click “Plan details” for more information or “Enroll in plan” to start the enrollment process.

Note: You must meet all Medicare health plan enrollment requirements. In Hawaii, you must also be a current member of the Kaiser Permanente Medicaid/QUEST plan.

In California, Hawaii, and Washington, Kaiser Permanente is an HMO plan with a Medicare contract. In Colorado, Kaiser Permanente is an HMO, HMO-POS and PPO plan with Medicare contracts. In Oregon, Southwest Washington, Georgia, Maryland, Virginia and the District of Columbia, Kaiser Permanente is an HMO and HMO-POS plan with a Medicare contract.
