Help with questionnaires

Questionnaires can be assigned to a Kaiser Permanente member for various reasons. They may be used to help prepare for an upcoming visit or procedure. A doctor may use a questionnaire to help manage a patient’s health. Or you or someone you care for may be given a questionnaire series to track progress during treatment or recovery after a procedure. Other questionnaires are optional.

Because questionnaires help us gather information needed to provide better care, we ask each patients to complete all questionnaires.

Please contact the doctor’s office and talk about any concerns.

Unfortunately, there isn’t a way to “opt in” to a questionnaire once you’ve selected the “opt out” option. Contact the doctor’s office and ask if they can send the questionnaire series again.
