About me

Hi, my name is Dr. Michael Allen. Welcome to my Web page. I am happy to have this opportunity to introduce myself and tell you a bit about my practice. I have been an optometrist with Kaiser Permanente since December 2006. Before I joined the Kaiser Permanente team, I had the great honor of serving four years as a Captain and optometrist in the U.S. Army. I was stationed for three years in Baumholder, Germany with the 1st Armored Division; my fourth year was spent at the Fort Irwin National Training Center right here in Southern California.

Biography

I was born and raised in Northern California. After completing a two-year church mission in Portugal, I returned to the U.S. with a desire to serve and work with people and decided to pursue a career as an optometrist. I earned my bachelor's degree from Brigham Young University in Hawaii, and my doctorate from Southern California College of Optometry in Fullerton. While attending optometry school I met the most beautiful young lady. We have been married since 2000 and have four children—three girls and one boy. They are my life and my greatest pleasure.

About my practice

I love working at Kaiser Permanente Fontana Medical Center, and I am fortunate enough to be able to give both optometry and ophthalmology care for our members. In our optometry department, I enjoy prescribing glasses and soft contacts; and when I provide members with ophthalmology care I have the opportunity to help patients manage or overcome ocular diseases. Working in the ophthalmology department allows me to use my therapeutic and glaucoma training and certificates. It also permits me to work closely with our great ophthalmologists, ranging from general to retinal specialists. If I cannot resolve your concern, I will be able to get you into the right hands here at Kaiser Permanente Fontana.

How I thrive

I thrive by focusing my life around my wife, kids and church. Every minute I spend with my family is a blessing and brings me great happiness. I try to stay healthy and active so I can enjoy my time with them. I am actively involved in my church, with my son's scouting program, and support my daughters in their extra-curricular activities.

Medicare Members: To request a hard copy of Kaiser Permanente’s provider directory, please call our Member Services department at 1-800-443-0815, seven days a week, 8 a.m. to 8 p.m. Kaiser Permanente will mail a hard copy of the provider directory to you within three (3) business days of your request. Kaiser Permanente may ask whether your request for a hard copy is a one-time request or if you are requesting to receive the provider directory in hard copy permanently.

If you request it, your request for hard copies of the provider directory remains until you leave Kaiser Permanente or request that hard copies be discontinued.

Medi-Cal Members: This directory includes Kaiser Permanente’s outpatient pharmacies. You can get any drugs covered by Kaiser Permanente at one of these pharmacies. You can get outpatient drugs covered by Medi-Cal at any Medi-Cal Rx Pharmacy. It does not have to be a Kaiser Permanente network pharmacy.  Most Kaiser Permanente network pharmacies are Medi-Cal Rx pharmacies. Your pharmacy can tell you if it is part of the Medi-Cal Rx network. Kaiser Permanente uses the same quality, member experience, or cost-related measures to select practitioners and facilities in Marketplace Silver-tier plans as it does for all other Kaiser Foundation Health Plan (KFHP) products and lines of business. The measures may include, but are not limited to, HEDIS/CAHPS performance, member/patient complaints, patient safety scores, hospital quality measures, and geographic need. Members enrolled in KFHP Marketplace plans have access to all professional, institutional and ancillary health care providers who participate in KFHP plans’ contracted provider network, in accordance with the terms of members’ KFHP plan of coverage. All Kaiser Permanente Medical Group physicians and network physicians are subject to the same quality review processes and certifications.
 
Kaiser Permanente uses the same geographic distribution consideration to select hospitals in Marketplace plans as it does for all other Kaiser Foundation Health Plan (KFHP) products and lines of business.
