Ready to enroll?

If you’re ready to enroll in a 2025 Kaiser Permanente Medicare health plan for individuals, you can enroll online or download and print our paper enrollment form.

You’ll need to enroll during your group’s specific enrollment period. If you don’t know when that is, ask your group’s benefits administrator.

If you have any questions, call 1-877-231-4833 (toll free) (TTY 711). Please mention to the Kaiser Permanente Medicare specialist that you will be losing your group coverage through your employer or union.

Some online services are not available in all areas.

You must reside in the Kaiser Permanente Medicare health plan service area in which you enroll.

In California, Hawaii, and Washington, Kaiser Permanente is an HMO plan with a Medicare contract. In Colorado, Kaiser Permanente is an HMO, HMO-POS and PPO plan with Medicare contracts. In Oregon, Southwest Washington, Georgia, Maryland, Virginia and the District of Columbia, Kaiser Permanente is an HMO and HMO-POS plan with a Medicare contract. Enrollment in Kaiser Permanente depends on contract renewal.
