American Heart Association
The Fremont and San Leandro Medical Centers earned Get With The Guidelines – Quality Achievement Awards from the American Heart Association. Each were awarded a Stroke Gold Plus with Target: Stroke Honor Roll Elite Plus and Target: Type 2 Diabetes Honor Roll and Heart Failure Silver Plus with Target: Heart Failure Optimal and Target: Type 2 Diabetes Honor Roll. Fremont also achieved Coronary Artery Disease STEMI Receiving Gold with Target: Type 2 Diabetes recognition.

Located in our Health Living Centers. Available at the Fremont Medical Center Monday through Friday 9 to 11 a.m. by drop-in or appointment. Available at the San Leandro Medical Center Monday through Friday 9 to 11 a.m. and 1:30 to 4 p.m. and Saturday 9 to 11 a.m. by drop-in or appointment. Available at the Union City Medical Offices Monday through Saturday 9 a.m. to 12 p.m. by appointment only.
