Help with updating your medical record

If a certain allergy, immunization, ongoing health condition, or past office visit you've had at Kaiser Permanente is not listed on our website, it is possible that this information is not included in your medical record. Please note that some of our online features may only present limited information from your medical record.

If the medical record information displayed on our site is incorrect or incomplete, you may download a form to request an update. To submit your request, please follow the instructions on the form.

You can use this feature to request a correction or addition to the health record information that displays on this website. For example, you can request to update the date of one of your immunizations if it is incorrect or ask that a drug allergen diagnosed by a practitioner be added to your list of allergies.

You cannot use this form to change contact information, ask about your health plan, or ask a nonurgent question about your health.

To update your address, change your preferred name, change your password or user ID, or update your e-mail profile, or to ask questions about your health plan, visit our member assistance center.

For nonurgent questions about your health conditions or care, please contact your local Kaiser Permanente facility. For departments and phone numbers, find doctors and locations.

One of our medical record office staff members will respond to your request in writing within the time frame indicated on the previous page. If more time is needed for us to respond to your request, we may ask for additional time, but will inform you of the delay first in writing.
