To view your eligibility and the status of your coverage, you must be signed on to kp.org.

Once you’re signed on:

  • Select “Benefits” from the dashboard.
  • Under “Plan Ahead,” select “Eligibility and benefits.”
  • View your eligibility summary, which includes your coverage effective and termination dates.

California American Specialty Health provides chiropractic services and discounts for Kaiser Permanente members. If your Kaiser Permanente plan includes chiropractic benefits, all covered services must be received from an American Specialty Health participating chiropractor. If your plan does not include chiropractic benefits, American Specialty Health has chiropractors that provide discounts to Kaiser Permanente members.

Colorado Kaiser Permanente’s Centers for Complementary Medicine provides chiropractic services for members with chiropractic benefits and fee for service for members and nonmembers that do not have chiropractic benefits. A referral is not necessary to visit Kaiser Permanente’s Centers for Complementary Medicine. Find locations and contact information for chiropractors on the Kaiser Permanente Complementary Medicine page or find contracted providers on the Find Doctors and Locations page.
Hawaii American Specialty Health provides chiropractic services and discounts for Kaiser Permanente members. Mid-Atlantic States If your plan includes chiropractic benefits, contact your primary care physician for a referral. If a referral is authorized, you can make an appointment with the chiropractor you are referred to.
Oregon/Washington The CHP Group provides chiropractic services to Kaiser Permanente members with plans that include chiropractic benefits. Depending on your plan, you can either self-refer to a chiropractor or a referral from your primary care physician may be required.

For plans with self-referred chiropractic benefits:

First, select a chiropractor from the CHP Group network or call the CHP Group at 503-203-8333. 

Once you have selected a chiropractor, call the chiropractor directly to make an appointment. Provide the chiropractor’s office with your Kaiser Permanente Health/Medical Record number and inform them that you have benefits through the CHP Group. The chiropractor’s office will contact CHP group to verify eligibility and benefits. At your appointment, pay any copays and coinsurance for services and/or X-rays.

The chiropractor’s office will submit a claim for the cost of the covered services to CHP Group. You are responsible for payment for any services received that are not covered.

For plans with physician-referred chiropractic benefits:

If your primary care physician includes chiropractic care in your treatment plan, you must select a chiropractor from the CHP Group network. Search for a chiropractor online or call the CHP Group at 503-203-8333. You will receive an authorization for the chiropractor you selected. Once you receive the authorization, call the chiropractor directly to set up an appointment. At your appointment, pay any copays and coinsurance to the chiropractor’s office for services and/or X-rays. The chiropractor’s office will submit a claim for the cost of the covered services to Kaiser Permanente. You are responsible the cost of any services not covered.

If you are enrolled in the DeltaCare® USA program Delta Dental of California will assign you a contract dentist based on your home ZIP code. Covered services must be received from your assigned contract dentist except for certain authorized referrals to a specialist and emergency services. For questions about your dentist, or if you would like to change your dentist, call the Delta Dental Customer Service number:
