Federal plans | Download, print, and return an Enrollment Change Form. To download a copy of the form visit the brochures and Forms page at the Kaiser Permanente Federal Employees site. To have a paper copy of the Federal Employees Enrollment Change Form mailed to you, send a message to Member Services through the kp.org message center. |
Medi-Cal plans | All changes to your membership information must be submitted Medi-Cal by calling or visiting the local county social services office. Medi-Cal will process any changes and then notify Kaiser Permanente of the updated information. |
Medicaid | All changes to your membership must be submitted to Medicaid by contacting your state or county Medicaid agency. |

All | In California, Colorado, Georgia, Maryland, Oregon, and Virginia: A newborn is covered under the parent’s coverage for 31 days, including the date of birth. You must request to add a newborn onto the account. (See instruction below). Forms completed at the time of birth do not enroll the newborn onto the plan. In Washington State: A newborn is covered under the parent’s coverage for 21 days, including the date of birth. You must request to add a newborn onto the account. (See instruction below). Forms completed at the time of birth do not enroll the newborn onto the plan. In Hawaii: A newborn is covered under the parent’s coverage during the birth month, if you add the newborn onto the account within 31 days. (See instruction below). Forms completed at the time of birth do not enroll the newborn onto the plan. |
Group plans | You must add the newborn or adopted child to the plan through the employer within 31 days of the date of birth or adoption. If you do not enroll the child through the employer within 31 days of the date of birth or adoption, you must wait until the next open enrollment period or qualifying event to add the dependent to the group plan. For more information about how to enroll your dependent, please contact your employer’s Human Resources department. Once you have completed the employer’s enrollment process, the employer will submit the requested changes to Kaiser Permanente. |

You may add the new dependent by completing and return an Account Change Form (available below)within 60 days of the birth or adoption. What should I do if I’ve moved recently or will be moving soon?
Kaiser Permanente offers coverage in California, Colorado, Georgia, Hawaii, Maryland, Oregon, Virginia, Washington, and Washington D.C.; however, where you live will affect the type of health care coverage you receive and how much you pay each month. If you're looking for a new plan in a new location, you can view plans at buy.kp.org or contact us at 1-800-295-8660 or 711 (TTY).
If you’ve recently moved or will be moving soon, complete the following steps to continue your new healthcare coverage.
1. Update your address, depending on your new type of coverage.
If receiving Medicare coverage:
- Call the Social Security Administration at 1-800-772-1213 or 711 (TTY).
- Visit the My Social Security website at www.ssa.gov. or
- Visit your local Social Security field office. The address can be found on the Social Security Administration website.
If receiving coverage through an employer:
- Notify your employer or their Human Resource Department of your new address.
If receiving federal or state marketplace coverage:
- Notify the exchange of your new address.
If receiving coverage purchased directly from Kaiser Permanente:
- Visit buy.kp.org or contact us at 1-800-295-8660 or 711 (TTY).
Note: If you’ve moved to a new state or region, you’ll be issued a new Medical Record Number (MRN).
2. Visit kp.org to create a new Kaiser Permanente online account and link your new account to your old account. Once your new account is activated, you’ll be guided through selecting your new primary care doctor and transferring any prescriptions and medical records.
Remember to enter the regional appointment and advice phone number for your new location in your cell phone.
