Help with COVID-19 status and Vaccine Certificate

When you have a COVID-19 test at a Kaiser Permanente facility, this status page will pull your test date and status information directly from your medical record.

If you get tested at a non-Kaiser Permanente medical facility, those test dates and results will not appear on this page.

When you get the COVID-19 vaccine at a Kaiser Permanente facility, your status page will show your vaccination date and information. This will include a reminder for a second vaccination if your first shot was Moderna or Pfizer.

If you received your vaccination at a non-Kaiser Permanente facility, pharmacy, or other location, those dates and status will not automatically be shown on this page. If you want, you can request for this information to be added to your Kaiser Permanente medical chart.

If you’ve had the first of a 2-shot vaccination outside of Kaiser Permanente, you must return to that original medical facility for your second shot.

In the same way, if you got your first shot at Kaiser Permanente, you should return to Kaiser Permanente for your second vaccination.

Your entitlements as a caregiver or parent are the same for this information as it is for any medical record information for those you care for.

If you think any information is missing or incorrect on your status page, there are several possible reasons.

  • There may have been a delay in including it in your medical record.
  • Tests received at non-Kaiser Permanente facilities won’t appear on your status page.
  • Vaccinations received at non-Kaiser Permanente facilities can be added to your medical record. You can request this update to complete your COVID-19 records.

A SMART Health Card contains a specific part of your medical record information such as your COVID-19 test results or vaccination history. It allows you to keep a copy of your records on hand and easily share this with others as you choose.

For example, you may use your SMART Heath Card to show your vaccine status for school registration or for travel. If you are a parent or caregiver, you can keep SMART Health Cards for yourself or for others, just as you would any other medical information.

Businesses, schools, and venues can verify an individual’s vaccination status by scanning the SMART Health Card’s two-dimensional bar code, more commonly known as a QR code. QR codes contain the same information as paper records, but in a format that offers greater security and efficiency. Information from QR codes is only available if and when you choose to share it.

To allow someone to verify your COVID-19 information, all you have to do is show them the QR code from your SMART Health Card. You can do this from a printout, or from the screen of your smartphone or by using an app on your phone to send them a digital copy.
