Our contracted providers in Northern California

Some Kaiser Permanente services are provided by contractors, which may include physicians, hospitals, and pharmacies.

Contracted practitioners and providers

In addition to the providers listed on our site, Kaiser Permanente is required by California law to provide members and prospective members a list of specific provider types contracted to provide outpatient behavioral health, acupuncture, and chiropractic services to Kaiser Permanente members.

Contracted behavioral health services provided by Carelon (formerly Beacon Health Options) are available by visiting their online provider directory.

Contracted chiropractic and acupuncture services provided by American Specialty Health Inc. (ASH) are available by visiting their online provider directory.

The information in these provider lists is updated periodically in our attempt to provide you with reliable and current information. The availability of physicians, providers, and services may change. This is not a guarantee of coverage. Please check with your Benefits Coordinator, Evidence of Coverage, or call Member Services at 1-800-464-4000 (toll free) or 711 (TTY for the hearing/speech impaired) to determine whether you have covered benefits. A referral may be required for some providers.

Printed contracted practitioners and providers list

Kaiser Permanente is required by California law to provide members and prospective members, upon request, a list of medical practitioners and providers contracted to provide health care services to Kaiser Permanente members in a general geographic area.

The list includes certain information about these contracted health care practitioners and providers, including which primary care practitioners may be accepting new patients. The list does not include those providers found on kp.org through the doctors and locations search tool. The Contracted Practitioners and Providers List is not intended to replace other Kaiser Permanente physician directories, provider listings, or guidebooks.

To obtain a printed list for any other area, contact Member Services and ask for the Contracted Practitioners and Providers List.

You may also request the Contracted Providers List by mail at the following address:

Family planning

State law requires us to include the following notice:

"Some hospitals and other providers do not provide one or more of the following services that may be covered under your plan contract and that you or your family member might need: family planning; contraceptive services, including emergency contraception; sterilization, including tubal ligation at the time of labor and delivery; infertility treatments; or abortion."

"You should obtain more information before you enroll. Call your prospective doctor, medical group, independent practice association, or clinic, or call Member Services at 1-800-464-4000 (English) or 1-800-777-1370 (TTY for the hearing/speech impaired), to ensure that you can obtain the health care services that you need."

Please be aware that if a service is covered but not available at a particular plan facility, we'll make it available to you at another facility.
