Help with immunizations

You’ll get the immunizations that health care practitioners have documented, including the name of the immunization and the date it was given. Keep in mind, not all immunizations may be listed.

Contact a local Kaiser Permanente facility to get a child’s immunization record. See our facility directory for contact information.

Select the name of an immunization, for example, influenza. You’ll get links that lead to in-depth information about the immunization. You can also see our health encyclopedia.

Note: If you live in Oregon or Washington state, use our health encyclopedia because direct links are not available.

If an immunization is missing, it’s possible that the immunization is not included in the medical record. Please see the information at the end of the list of immunizations to find out how to request a medical record update.
