About me

Welcome! My name is Dr. Tahmina Rahman (pronounced Tah-mee-nah Rah-maan).

 

Biography

 

I’m grateful for the opportunity to share a little about myself with you. I believe it’s important that you feel comfortable and confident with your personal physician, and I hope this page gives you a chance to get to know me better. I was born and raised in Bangladesh, where I began my medical career before moving to the United States to continue my training and education. I have earned a master’s in global medicine from USC. I completed my residency in family medicine and have been practicing with Kaiser Permanente since 2016.

 

I chose family medicine because I value the opportunity to build long-term relationships with members and care for them through all stages of life. I enjoy working with members on preventive health, chronic disease management, and empowering them to take an active role in their well-being. Beyond my clinical work, I’ve had the privilege of serving in various leadership roles at Kaiser Permanente, including as a hypertension champion, depression care management champion, and peer reviewer. I also participate in community and quality improvement initiatives that aim to improve the health of our members and the communities we serve.

 

About my practice

 

Southern California has become home for me and my family, and in my free time I enjoy reading, traveling, and spending time with loved ones. I believe in practicing medicine as a true partnership between physician and member. My role is not only to diagnose and treat illness, but also to guide and support you in making informed choices about your health. I emphasize preventive care and the management of chronic conditions, as I find that small, consistent steps can lead to lasting improvements in well-being.

 

I value listening to members and understanding their unique experiences because I believe good care comes from trust, respect, and teamwork. My goal is to provide compassionate care based on evidence and current research that helps you and your family live healthier, fuller lives.

 

How I thrive

 

Keeping balance in my life is very important to me. I enjoy spending time with my family, traveling, and exploring new cultures. At home, I find joy in gardening and cooking, which allow me to be creative and stay connected to a healthy lifestyle. Reading and writing also help me recharge, and I make it a priority to stay active through regular exercise, as well as practicing mindfulness and gratitude.

Medicare Members: To request a hard copy of Kaiser Permanente’s provider directory, please call our Member Services department at 1-800-443-0815, seven days a week, 8 a.m. to 8 p.m. Kaiser Permanente will mail a hard copy of the provider directory to you within three (3) business days of your request. Kaiser Permanente may ask whether your request for a hard copy is a one-time request or if you are requesting to receive the provider directory in hard copy permanently.

Medi-Cal Members: This directory includes Kaiser Permanente’s outpatient pharmacies. You can get any drugs covered by Kaiser Permanente at one of these pharmacies. You can get outpatient drugs covered by Medi-Cal at any Medi-Cal Rx Pharmacy. It does not have to be a Kaiser Permanente network pharmacy.  Most Kaiser Permanente network pharmacies are Medi-Cal Rx pharmacies. Kaiser Permanente uses the same quality, member experience, or cost-related measures to select practitioners and facilities in Marketplace Silver-tier plans as it does for all other Kaiser Foundation Health Plan (KFHP) products and lines of business. The measures may include, but are not limited to, HEDIS/CAHPS performance, member/patient complaints, patient safety scores, hospital quality measures, and geographic need. Members enrolled in KFHP Marketplace plans have access to all professional, institutional and ancillary health care providers who participate in KFHP plans’ contracted provider network, in accordance with the terms of members’ KFHP plan of coverage. All Kaiser Permanente Medical Group physicians and network physicians are subject to the same quality review processes and certifications.
 
Kaiser Permanente uses the same geographic distribution consideration to select hospitals in Marketplace plans as it does for all other Kaiser Foundation Health Plan (KFHP) products and lines of business.
