What is a Dual Eligible Special Needs Plan (D-SNP)?

If you have Medicare and Medicaid (Medi-Cal in California), you may hear the term “dual eligible”. It simply means you qualify for both programs.

If you're dual eligible, you may also qualify for a special health plan called a Dual Eligible Special Needs Plan, or D-SNP. This plan brings your Medicare and Medicaid/Medi-Cal benefits together and does not replace them. It also gives you extra help and can make it easier to get the care you need.

Here’s how each one helps you:

  • Medicaid/Medi-Cal helps pay for more health costs and other care, like long-term care.
