Help with authorization to share records

To authorize access for specific providers, check the boxes next to your preferred organization and select Continue.

To authorize access for all providers participating in HIE with Kaiser Permanente, select the All health providers button and click Continue. Note that Kaiser Permanente will only share your records with an organization that also has treating relationship with you.

After selecting Continue, review the authorization form and select Authorize.

The organization may not be available for electronic authorization at this time or may not be part of the Health Information Exchange network. If you need to send your medical records to that organization, contact the Member Services number on the back of your Kaiser Permanente ID card and request that your records be sent.

For Kaiser Permanente members in Oregon/Washington: Authorizations are permanently valid for your lifetime.

For Kaiser Permanente members in all other regions: Authorizations will expire after one year.

After one year, you will again need to authorize HIE access for your non-Kaiser Permanente providers and Kaiser Permanente providers in other regions.

For Kaiser Permanente members in Oregon/Washington: Authorizations are permanent and do not need to be extended.

For Kaiser Permanente members in all other regions:

  • Check the box next to the organization you would like to extend and select “Continue.”
  • Review the authorization form and select “Authorize.”
  • This will extend HIE access for that organization another year.

You may have previously authorized access via a paper authorization form either at Kaiser Permanente or at the organization requesting your Kaiser Permanente records.

If you never provided authorization, contact the Member Services number on the back of your Kaiser Permanente ID card.

To remove authorization for a provider to access your information through the Health Information Exchange network, send a written request to Kaiser Permanente, Release of Information Department at:

Authorization is only required for members with information from a substance use disorder program in their records.

Per the Health Insurance Portability and Accountability Act (HIPAA), written authorization is not required for other members to participate in HIE. This is because the information is being shared between your care providers to assist in your treatment. Electronic exchange of your information allows your health care providers to obtain a more complete picture of your medical history and have it all in one place, reducing the likelihood of conflicting diagnoses, prescriptions, and treatment plans as well as repeated tests or procedures.
