We know that a behavioral health crisis is a vulnerable time. Our emergency department staff is trained to give holistic care to patients in crisis.

During the visit, emergency department staff will encourage the patient to identify a supportive person or caregiver who can help maintain safety and provide support and encouragement after discharge.

The staff will also assess the patient's ability for self-care and ability to return safely to the home environment. Staff will identify available community services of benefits to the patient. Trained staff will complete a behavioral health assessment, including:
  • A suicide risk assessment
  • Medical, psychological, and functional assessments

Patients will receive comprehensive discharge plans that:

  • Support medical, functional, and psychosocial needs agreed upon by both patient and staff
  • Coordinate with primary care providers
  • Include referrals to peer support resources
  • Address other relevant follow-up issues
  • Detail a safety plan specific to self-harm or suicide prevention

To support our patient's continued safety:

  • We will work to provide outreach and attempt to schedule a follow-up appointment within 7 days of release from the emergency department
  • Contact will be attempted by a team member within 48 hours of release from the emergency department for patients with identified safety risks

Kaiser Permanente may ask whether your request for a hard copy is a one-time request or if you are requesting to receive the provider directory in hard copy permanently.

If you request it, your request for hard copies of the provider directory remains until you leave Kaiser Permanente or request that hard copies be discontinued.

All Kaiser Permanente Medical Group physicians and network physicians are subject to the same quality review processes and certifications.

Referrals to affiliated community physicians/providers in Oregon and Washington

Affiliated community physicians/providers are not part of the NWPMG (Northwest Permanente Medical Group). They are physicians/providers credentialed by Kaiser Foundation Health Plan of the Northwest and/or who have a contract with the NWPMG.

If your NWPMG physician decides that you require covered services not available internally from an NWPMG physician, he or she may refer you to an affiliated community physician/provider. You must have a written, authorized referral from Kaiser Permanente in order to receive a covered service from an affiliated community physician/provider.

Referrals to affiliated community dentists in Oregon and Washington

Affiliated community dentists are not part of PDA (Permanente Dental Associates). They are dentists credentialed by PDA for Kaiser Foundation Health Plan of the Northwest, and they have a contract with PDA.

If your PDA dentist decides that you require covered services not available internally from a PDA dentist, he or she may refer you to an affiliated community dentist. You must have a written, authorized referral from Kaiser Permanente in order to receive a covered service from an affiliated community dentist.
