How to report concerns

Kaiser Permanente encourages you to report any concerns you have about your safety, quality of care, or privacy, as well as any suspected fraud.

 

Reporting concerns about quality of care

If you have any concerns about your safety or quality of care, please talk about them with your health care team. If you don’t understand the answers you receive, ask for more information.

If your concerns cannot be resolved with your health care team, we encourage you to report your concerns to the management team at your medical center. Find contact information for your medical center online, or contact Member Services for assistance.

 

Reporting concerns to The Joint Commission

If your concerns about quality of care during a hospital stay cannot be resolved through managers at the hospital, we encourage you to contact The Joint Commission, an independent, not-for-profit organization that accredits and certifies health care organizations and programs in the United States.

 

Reporting a suspected privacy breach

We take the trust you put in Kaiser Permanente seriously and continue to do all we can to keep your medical health information private, protected, and secure.

If you believe that the privacy of your medical record may have been compromised, please contact Member Services.

 

Reporting suspected fraud

We encourage our members, as well as our vendors and others, to let us know of any situation at Kaiser Permanente that may be unlawful. If we know about such a situation, we can investigate and take action to protect our members and our health care resources.
