Important privacy notice for some Kaiser Permanente members

Kaiser Foundation Health Plan of Northwest (“Kaiser Permanente”) is posting this notice to inform its members about a privacy matter that may have affected them. We are contacting affected members directly via U.S. Mail to notify them of the incident described below. If you did not receive a notification letter and have questions about whether you were affected, you can contact Kaiser Permanente at the contact information provided at the end of this notice.

What Happened:
On April 24, 2024, Kaiser Permanente determined that a system error caused the billing notices of approximately 580 members in Oregon to be mailed to the wrong address. As a result, the personal information of these members might have been sent to an unintended recipient. We apologize that this incident occurred.

What Information was Involved:
The information that may have been involved was limited to first and last name, member number, amount due, coverage date and rates, and potentially the name(s) of dependent(s). Social Security numbers and credit card information were not included in the information.

What We Are Doing:
After discovering the error on April 24, 2024, we began a comprehensive analysis to identify the cause of the problem. We have put measures in place to avoid future situations like this from occurring.

What You Can Do:
Kaiser Permanente is not aware of any misuse of your information. If you are concerned about identity theft and would like more information on ways to protect yourself, visit the Federal Trade Commission’s Identity Theft website at https://www.identitytheft.gov.
