The Anticoagulation Service is also known as Warfarin or Coumadin Clinic. It is a telephone only service.
The Anticoagulation Service is dedicated to help your physician manage your anticoagulation therapy and provide you with comprehensive information regarding anticoagulation medications, mainly warfarin. You will also be invited to attend warfarin class upon enrollment.
Medication Safety: Many factors can affect the activities of anticoagulation medications. Please call us at 408-366-4323 to inform us of any changes in your medications, diet, or your health condition since these changes may alter your anticoagulation blood test results. It is for your safety that we ask you to call us when you have the above changes.

Medicare Members: To request a hard copy of Kaiser Permanente’s provider directory, please call our Member Services department at 1-800-443-0815, seven days a week, 8 a.m. to 8 p.m. Kaiser Permanente will mail a hard copy of the provider directory to you within three (3) business days of your request. Kaiser Permanente may ask whether your request for a hard copy is a one-time request or if you are requesting to receive the provider directory in hard copy permanently.

Parking and Valet Information
Please arrive 30 minutes before your scheduled appointment to allow for ample time for parking. There will be times when parking can be difficult. For that reason, we encourage you to use the Valet Parking Service located at Homestead West Lobby of the Medical Office Building. There is no cost to this service and no gratuities are expected or accepted by the valets. You can even call ahead to have your car available for your departure from the medical center.
