Large group plan documents

Kaiser Permanente's Point-of-Service (POS) Plans are underwritten by Kaiser Foundation Health Plan of Colorado (KFHP of CO) and Kaiser Permanente Insurance Company (KPIC). In a 3-tiered POS Plan, the HMO In-Network Provider Tier is underwritten by KFHP of CO; and the Participating Provider and the Non- Participating Provider Tiers are underwritten by KPIC, a subsidiary of KFHP. In a 2-Tiered POS Plan, the HMO In-Network Provider Tier is underwritten by KFHP of CO and the Non- Participating Provider Tier is underwritten by KPIC.

Kaiser Permanente's PPO Plans are underwritten by KPIC, a subsidiary of Kaiser Foundation Health Plan.
