One Pass® Fitness Program [See footnote 1]

One Pass can help you find a fitness routine that’s right for you, whether you work out at home or at a gym.

Select plans receive an annual (one calendar year) allowance of up to $500 to spend on covered health-related services. Covered items include:

  • Personal training fees.
  • Fitness class fees that do not come with a basic One Pass membership (e.g., yoga, Pilates).
  • Out-of-network fitness location fees (e.g., gyms, fitness studios, climbing gyms).
  • Additional class fees within the One Pass network that exceed the basic One Pass membership (e.g., power cycling).
  • Raquet club memberships (e.g., tennis, pickleball).

Members will be reimbursed for covered expenses. Any unused amounts do not roll over to the next year. Please see the EOC for more information.

How to get started with the One Pass program

Once you're a Kaiser Permanente Medicare health plan member, follow these steps: 

  1. Visit youronepass.com
  2. Click "Get Started" to register. Enter in your First Name, Last Name, Date of Birth and Health Plan Member ID. 
  3. Once you're registered, you'll receive a Member Code. Be sure to write down your code and keep it handy. You will need to enter it each time you register for a new fitness location or other One Pass service.
  4. Start searching for gyms by clicking on the "Find a gym" page. 

You may also call One Pass toll-free at 1-877-614-0618 (TDD/TYY 711), Monday through Friday, 6 a.m. to 7 p.m. Pacific time, to receive your Member Code.

The One Pass program is also available to Dual Complete (HMO D-SNP) plan members.

We cover routine eye exams (eye refraction exams) to determine your needs for vision correction and to provide a prescription for eyeglass lenses or contact.

Your supplemental benefits include an allowance to use toward the purchase of eyewear from a plan optical facility, such as Vision Essentials by Kaiser Permanente. A prescription is required.

Your allowance can be used to pay for the following items:

  • Eyeglass lenses and frames
  • Contact lenses, fitting, and dispensing.

This benefit allowance may also be increased if you are enrolled in Advantage Plus. See your Evidence of Coverage for more details and information on the allowance included with your Medicare health plan.

We cover OTC items listed in our OTC catalog for free home delivery. The catalog lists the price of each item. You can view our catalog and place an order online. You may also place an order over the phone or request a printed catalog be mailed to you by calling 1-833-238-6616 (TTY 711), 6 a.m. to 6 p.m., Monday through Friday.

The quarterly benefit limit varies by plan enrolled:

  • $70 quarterly benefit limit for Core plan members (Southern Colorado)
  • $75 quarterly benefit limit for PPO Choice plan members (Denver Metro and Southern Colorado)
  • $80 quarterly benefit limit for Core (Denver Metro) and Enhanced plan members
  • $85 quarterly benefit limit for Core (Northern Colorado) plan members
  • $90 quarterly benefit limit for PPO Choice (Northern Colorado) plan members
  • $100 quarterly benefit limit for Silver (Denver Metro), Gold, and Dual Complete (HMO D-SNP) plan and Dual Essential (HMO D-SNP) plan members 
  • $90 quarterly benefit limit for Gold plan members
  • $105 quarterly benefit limit for Silver (Northern Colorado) 
  • $150 quarterly benefit limit for Dual Complete (HMO D-SNP) plan members
  • $120 quarterly benefit limit for Bronze plan members
  • $70 quarterly benefit limit for employer groups

We cover routine hearing exams and fittings eye exams as well as fittings and evaluations for hearing aids.

All Kaiser Permanente Colorado Senior Advantage plans include hearing aid coverage. Your supplemental benefits include an allowance to use toward the purchase one hearing aid per ear every two years.

This benefit allowance may also be increased if you are enrolled in Advantage Plus. See your Evidence of Coverage for more details and information on the allowance included with your Medicare health plan.

We cover routine hearing exams and fittings eye exams as well as fittings and evaluations for hearing aids.

All Kaiser Permanente Colorado Senior Advantage plans include hearing aid coverage. Your supplemental benefits include an annual allowance to use toward the purchase of a hearing aid.

Kaiser Permanente Colorado Medicare members are eligible for non-emergency transportation to providers within the Denver Metropolitan, Northern Colorado, and Southern Colorado service areas each year at no cost.

This benefit provides a set number of one-way trips annually. Trips are within your respective service area and limited to 65 miles, one way. [See footnote 2]

  • Senior Advantage Gold (HMO-POS) plan: 40 one-way rides per year
  • Senior Advantage Bronze (HMO-POS) and Choice PPO plans: 18 one-way rides per year
  • Senior Advantage Core (HMO) (Denver Metro) plan: 12 one-way rides per year
  • Senior Advantage Core (HMO) (Southern and Northern Colorado) plan: 22 one-way rides per year
  • Senior Advantage Silver (HMO-POS) and Enhanced (HMO-POS) plans: 26 one-way rides per year
  • Dual Complete (HMO D-SNP) and Dual Essential (HMO D-SNP) plan: unlimited one-way rides per year
  • Medicare employer group (HMO) plans: 20 one-way rides per year
  • Advantage Plus: 20 one-way rides per year

Travel with peace of mind knowing that Kaiser Permanente Senior Advantage Bronze, Silver, Enhanced, and Gold (HMO-POS) plans include Medicare Explorer, a $1,000 annual allowance for out-of-area routine and continuing care. Medicare Advantage employer group plans include a $1,500 Medicare Explorer annual allowance (check your Evidence of Coverage to see if your plan offers). In-home support allows eligible Medicare health plan members to receive assistance with activities of daily living within the home. Kaiser Permanente has partnered with CareLinx, an online system that connects members and families with professional, non-medical caregivers in their area. You can access the portal to manage in-home care, find a caregiver, see information on upcoming services, and assign specific care requests to your caregiver for future visits. 

We cover 60 hours of non-medical, in-home support services every year to address assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) within the home. [See footnote 5] See the Evidence of Coverage for details.
