Your electronic health record connects your care team to your latest medical information. They can view your doctor visit notes and history anytime, so you don’t have to repeat yourself with different care team members. Instead, you can focus on what’s most important — getting well and staying well. Plus, you can view your health information anytime at kp.org or with our app.

Your electronic health record also connects your care team members to each other, so it’s easy for them to work together to keep you healthy. They measure and record your cardiovascular health at almost every appointment. This helps them spot problems earlier, so you can start treatment as soon as possible. 

Your care team has access to some of the most advanced cardiac procedures available. For example:

• Minimally invasive heart surgery: With our innovative transcatheter aortic valve replacement surgery, we can replace heart valves without open-heart surgery. For patients, this means shorter recovery times, smaller scars, and less time in the hospital. They can go home after 1 or 2 days on average, instead of a week or more after open-heart surgery.

• Home cardiac rehabilitation program: Our 8-week recovery program lets patients complete daily rehab activities at home, which their doctor can track remotely. This new option is combined with phone appointments and ongoing health education, and has already helped reduce hospitalization rates for patients. It’s currently available in Northern California, but we’ll be expanding it to other areas soon.
