It’s easy for you and your care team to keep track of your blood pressure — right from your home. With our home monitoring program, you can test your blood pressure and have the information automatically sent to your electronic health record, where your care team can view it.

How to get started

Once you have your blood pressure monitor and the app, follow these simple steps:

Step 1

Sign in to the KP Health Ally app: Use your kp.org user ID and password to sign in. If you haven’t created a kp.org account yet, tap “Register” on the sign-in page.

Step 2

Review the program information: Select the program your care team has assigned you in the app. You can read more about it by expanding each section. In the “How It Works” section, you’ll see the goals you’ve agreed to with your care team.

Step 3

Pair your device: Once you’ve enrolled, you’ll be asked to pair your device. Make sure Bluetooth is turned on in your mobile device settings. Then follow the instructions to pair it to your blood pressure monitor. When prompted, enter the code located at the bottom of the monitor.

Step 4

Track your blood pressure: The KP Health Ally app will let you know when your blood pressure monitor is ready to use. Keep in mind that it needs to be in Bluetooth range of your mobile device to record the readings. And be sure the app is open to send the information to your care team.
