The Plan of Care feature only shows goals for a specific condition, such as diabetes. If you have questions about your other health goals, speak with your doctor at your next office visit, or email your doctor.

The conditions may have different care options. You may not need to track your health for coronary artery disease, but may need to track your blood sugar level for diabetes care. Or your provider may not set goals for asthma but has set goals for hypertension.
