Health trackers, which your practitioner might call flowsheets, provide a way for you to help your doctor monitor your health. Through our website, you can enter information (such as your weight or blood pressure) in your health tracker from home. Your doctor can review and record the information in your medical record. Trackers are useful when you need to monitor your health for a certain period of time.

Using the health trackers feature begins with your doctor. Your doctor must order a health tracker for you before you can access the feature through our website. Once your doctor has ordered your tracker, you will receive an email notification. After that, you can sign on to our website any time to use your trackers. To request a new health tracker, please contact your doctor’s office.

A health tracker order may not always come directly from your doctor. The name on the order may be a nurse or other practitioner on your doctor’s team.

Yes, you can add readings, but only to your active health trackers.

If the reading has not yet been reviewed by your provider, you can select a link to edit or delete that reading. However, once your provider has reviewed a reading, it cannot be changed.

You can add new entries or change existing entries only to a health tracker that is active. Check to make sure that your tracker is under the Active health trackers list before trying to change the information. Once you select an active tracker, you can view entries for that particular tracker. From the list of entries (depending on your region), you can either add new entries or select an existing entry to edit.

Health trackers are active when you first receive them and remain active for a period of time defined by your doctor. You can enter and change information in your tracker only while it is active. Once the active period has passed, you will only be able to read the tracker. It is available for you to review, but can no longer be changed.

Enter your information into the health tracker using the standard units of measurement listed in the chart below. If you use different units of measurement (for example, if your scale measures weight in kilograms), contact your doctor for instructions. If you need more information on how to take a measurement or why, ask your doctor.

You may not have to enter information for all times listed on the health tracker. Ask your doctor how often you should enter your information.
