How to Qualify and Apply for Medicaid in Washington State
The steps below will help you understand how to apply for Medicaid in Washington State and get care through Kaiser Permanente for yourself or your child.

For information about applying and enrolling in Apple Health/Medicaid, please call the Customer Support Center at 1-855-WAFINDER / 1-855-923-4633.
In addition to your completed Medicaid application, you may be asked to provide other documents, such as:
Identification
Proof of U.S. Citizenship or Immigration Status
Proof of residency (you must live in the state in which you apply)
Income status
2. Receive approval for Medicaid.
Depending on how you apply, you can find out right away if you are eligible for Medicaid coverage. In some cases, it could take the state up to 45 days.
Once you are approved for Medicaid, you’ll receive a welcome packet from the state.
3. Select Molina Healthcare as your Managed Care Organization (MCO) and choose Kaiser Permanente as your healthcare provider.
Note: As of June 1, 2023, Kaiser Permanente Washington is closed to all new Molina Apple Health patients in the greater Puget Sound area (Snohomish, King, Pierce, Thurston, or Kitsap counties) and in Spokane county.
If you live in Southwest Washington (Clark/Cowlitz counties) and would like to choose Kaiser Permanente as your healthcare provider, you must select Molina Apple Health as your Managed Care Organization (MCO).
Once you’ve selected Molina as your MCO, you will receive a welcome packet and ID card from Molina in about two weeks.
Kaiser Permanente may or may not be accepting new patients/members depending on where you live and our contract with Molina.
If Kaiser Permanente is not accepting new patients/members, an exception may be available if:
The applicant is under age 19, OR
You have an immediate family member who is a Kaiser Permanente established patient, OR
Were or are a Kaiser Permanente established patient in the last 12 months
An immediate family member is a family member who lives in the home of a current Kaiser Permanente member and is a:
Spouse (including Domestic Partners)
An unmarried dependent child under age 21
A disabled dependent over the age of 21
A married/unmarried stepparent of children under age 21
A foster child or stepchild
A legal guardian
A grandparent, parent, guardian, or other relative who applied for Medicaid on behalf of a current Kaiser Permanente established patient that is under 21
4. Receive your Kaiser Permanente ID card(s) once you’re enrolled.
Please be sure to bring both your Molina ID card and your Kaiser Permanente ID card (if applicable) any time you receive services.
