Your Kaiser HMO Option

With your JPMorgan Chase Kaiser HMO Plan, you can expect a wide range of care and support to help you stay healthy, including no-cost preventive care, no-cost telehealth, and doctor’s office visits and prescriptions for a copay or coinsurance — even before you meet your deductible.

Explanation of Benefits are not a bill. They are a summary of services you’ve received. It shows the charges, the date of your visit, and the name of the provider you visited. Use them to:

  • Keep track of your expenses and make sure everything is accurate.
  • Check your progress; have you reached your deductible or out-of-pocket maximum?

Before your EOB arrives, use our estimates tool on kp.org to get the most current overview of how close you are to reaching your deductible and out-of-pocket maximum. For your JPMorgan Chase MRA and HCSA or DCSA be sure to save your EOBs in case you need to validate a claim or for income tax purposes.
