• Affiliate Provider Collaborative Meetings, Practitioners and Providers are expected to participate in Kaiser Permanente’s Quality Oversight Program. This may include collaboration or active participation on accreditation metrics, sharing of performance metrics, mitigation of performance through process improvement activities and monitoring for sustainability. • Effective immediately-- Kaiser Permanente Colorado is discontinuing the temporary allowance for long term care (LTC) facilities to provide Part B Therapy services to Kaiser Permanente Colorado members. These services can now be coordinated with Vitality Rehabilitation and Julia Temple Outpatient Services.
• Rehab Services Referral and Authorization Process - for contracted vendors refer to the Rehab Services Referral and Authorization Process FAQ Sheet.
• Required Provider Enrollment for those that see patients with Child Health Plan Plus (CHP+) and Health First Colorado (Medicaid)
• Important Update: Changes to Kaiser Permanente’s Telephonic Medicine Center’s Contact Procedures - Effective January 29, 2025, Kaiser Permanente is introducing a phone tree system to improve call tracking and triage efficiency. Please note the updated phone number to the Telephonic Medicine Center (TMC): 

Providers and departments external to Kaiser Permanente should begin contacting the TMC at (720) 462-4994.  The current contact of (303) 318-1111 will remain in effect for some time but will be phased out in 2025.
