Enrollment, Registration and Sign-On

There are two simple steps to gain access to the Online Affiliate portal. This includes:

Step 1: Enrollment

Completion of our Online Affiliate Enrollment Agreement is the first step in gaining access to Kaiser Permanente’s Online Affiliate portal for your individual practice, group, or facility. The agreement must be signed by an individual authorized to sign legal documents on behalf of the organization.

This agreement outlines the conditions and responsibilities the authorized signatory must assume for any individual user who associates themselves with the individual practice, group, or facility. Be advised, we only require one agreement per Tax ID.

Unsure if we have an agreement on file for your entity?

Submit an Online Affiliate Support Case with your group name and Tax ID, and a consultant will respond to confirm if we have an agreement on file.

Please read, sign, and return the agreement using our webform:

Proceed to Step 2 once you have received confirmation that your Online Affiliate Enrollment Agreement has been processed.

A Third-Party Administrator (TPA) is a firm or organization that manages administrative functions, such as claims processing, for another health company.

TPAs may request administrative/non-clinical access to KP Online Affiliate by submitting the following agreements:

Online Affiliate Enrollment Agreement (Must be completed by the TPA)

TPA Attestation (Must be completed by the Provider Group)

Please return the agreements using our webform:

Proceed to Step 2 once you have received confirmation that your enrollment request has been processed.

Step 2: User Registration

Once you have completed Step 1 and have received a confirmation that your National Enrollment Agreement has been processed, the next step is to register as a user to access Online Affiliate.

Take a look at our User Registration Guide to get familiar with the process and then begin registration by proceeding to the New User Registration Portal.
