Dental coverage

 

Some Kaiser Permanente plans offer coverage for adult or pediatric dental care. If your plan includes dental coverage, or if you have selected an optional dental plan, click on your plan type below to learn how to use your benefits.

Does my plan include dental coverage?

If you're not sure if you have dental coverage, or what plan you have, call Kaiser Permanente Member Services at 206-630-4636 or 1-888-901-4636, Monday through Friday, 8 a.m. to 5 p.m.

Medicare Advantage Individual plan
If you’re a Kaiser Permanente Medicare Advantage (HMO) health plan1 member, your plan includes a preventive dental care benefit, that covers preventive and diagnostic dental services at no cost to you. Or you may have enrolled in comprehensive dental coverage by selecting the Advantage Plus optional supplemental Delta Dental PPO Plus PremierTM plan for an extra monthly premium. Both benefits are provided by Delta Dental of Washington and use the Delta Dental PPO Plus PremierTM network. Dental Member Services (Delta Dental of Washington)
If you have questions about your dental benefit usage and providers, call Delta Dental of Washington Customer Service at 1-877-719-4006 (TTY 711), Monday through Friday, 7 a.m. to 5 p.m. You can also register your MySmile® account at DeltaDentalWA.com get dental coverage details, preventive care reminders, treatment costs, and more.

Your ID number
You will not receive a separate ID card or identification number for your dental benefits. When you receive dental care, provide your Delta Dental network dentist with your Kaiser Permanente medical plan subscriber's member ID number, which appears on your Kaiser Permanente member ID card. Your Delta Dental network dentist should use your Kaiser Permanente medical plan subscriber’s member ID number when submitting claims to Delta Dental of Washington.

Finding a Delta Dental network dentist
Visit DeltaDentalWA.com and use the Find a Dentist tool. Remember to choose the Delta Dental PPO Plus PremierTM network. The online directory is easy to use anytime, at home or on your smartphone. You can search based on preferences that matter to you, including dentist name, specialty, location, and language. You can even see endorsements from other Delta Dental patients for categories including extended office hours, friendly staff, and if they help ease anxiety. You can also call Delta Dental at 1-877-719-4006 (TTY 711) for assistance finding a network dentist.

Federal Employees Health Benefits plan (FEHB)
If you're a Federal Employees Health Benefits Plan (FEHB) member and have selected one of the Kaiser Foundation Health Plan of Washington Options, Inc., plans (PPO), your medical plan includes preventive dental coverage. Visit the Kaiser Permanente Washington Options Federal plans page  or call Kaiser Permanente Member Services toll-free at 1-888-901-4636 for information about your dental benefits.
Individual and Family Plans

If you’re an Individual and Family plan member, your dental coverage is provided by Delta Dental of Washington and uses the Delta Dental PPO Plus Premier™ network. Please visit the Kaiser Permanente Washington Individual and Family page for more information.

 

Dental Member Services (Delta Dental of Washington)

Your ID number
Your Delta Dental coverage doesn’t have its own identification number. When you or a family member covered on your plan receives dental care, use the Kaiser Permanente medical plan subscriber’s member ID number, which appears on the subscriber’s Kaiser Permanente member ID card. If you’re not sure who the subscriber is for your plan, call Kaiser Permanente Member Services at 1-800-290-8900 (TTY 711).

Visit a participating Delta Dental network dentist
To get the most from your benefits, we encourage you to see a participating dentist. These dentists contract with Delta Dental to provide services at discounted fees and file all claims for you. Dentists who are part of Delta Dental’s networks will not charge more than their approved fees and usually cost you less than out-of-network dentists. You may choose any licensed dentist to provide services under this plan. However, if you go to an out-of-network dentist, Delta Dental has no control over their fees. You will be responsible for submitting claims and paying any difference in the charges. This is called balance billing.

Choosing your plan
You can choose from 2 kinds of dental plans.

Adult/Family Basic plan

The optional Adult/Family Basic plan includes dental coverage for everyone covered on the medical plan.

  • This dental plan is available for adults or families who buy their medical plan directly from Kaiser Permanente.

  • Adults or families who purchase their medical coverage through Washington Healthplanfinder may also purchase a dental plan on the exchange.

Pediatric plan

  • The pediatric plan offers dental coverage for those 18 and younger. The state of Washington requires you to have pediatric dental coverage as part of your medical plan if you have dependents 18 and younger.
  • This dental plan is available if you buy your medical plan directly from Kaiser Permanente.
  • If you sign up for a pediatric dental plan with another company, complete an Attestation of Pediatric Dental Coverage form and send it to us along with proof of dental coverage within 60 days of the start date of your medical plan.
  • If you don’t submit this information within the 60 days, you won’t meet the minimum health and dental coverage required by Washington state and federal regulations.

Small Group Plans

If you’re a Small Business Group plan member (covered by an employer with 50 or fewer employees), your dental coverage is provided by Delta Dental of Washington and uses the Delta Dental PPO Plus Premier™ network. Visit the Kaiser Permanente Washington Small Group page for more information.

Large Group plan (Health360)
If you're a Large Group plan member (covered by an employer with 51 or more employees) on the Health360 plan, your dental coverage is provided by Delta Dental of Washington and uses the Delta Dental PPO® and Delta Dental Premier® networks.

Your ID number
Your Delta Dental coverage comes with a unique identification number. When you or a family member covered on your plan receives dental care, you will use your Delta Dental ID card. To view a digital version of your ID card, visit deltadentalwa.com and register for a MySmile® account.

Your dental plan
Your Health360 dental plan offers the following benefits:

  • An annual maximum step increase that you earn for each year receiving preventive dental care from a PPO network dentist. You may repeat this each year until you reach your annual maximum upper limit.
  • An extra dental cleaning and exam with any PPO network provider are offered every year, at no additional charge, to patients with qualifying conditions, including heart disease, diabetes, and pregnancy. Patients who qualify for the extra cleaning and exam will be notified by mail.
  • An option to sign up for dental care reminders via email through your MySmile® account.

Choosing an in-network dentist
Your plan gives you access to the Delta Dental PPO® network. Visiting a Delta Dental PPO dentist helps maximize your benefits and reduce your out-of-pocket costs. A list of Delta Dental Premier® and Delta Dental PPO® dentists is available through Delta Dental’s Find a Dentist tool.

Claims and billing
Your PPO network dentist will submit claims to Delta Dental of Washington on your behalf. If you have questions about a claim, call Delta Dental Customer Service at 1-800-554-1907, Monday through Friday, 7 a.m. to 5 p.m.
