About this program
Any Kaiser Permanente member with Type 2 Diabetes who are either newly diagnosed or interested in learning more about managing their Diabetes. This class is not appropriate for those with Type 1 or Gestational Diabetes.
• Facilitated by a registered dietitian (RD) and a registered nurse (RN) who specialize in diabetes care.
• Learn practical strategies for putting nutrition recommendations into action in real-life situations.
Class 1: intro to diabetes, nutrition balance, the plate method, SMART goals
Class 2: meal planning, food labels, monitoring glucose, hemoglobin A1c, physical activity
Class 3: medications, shopping and dining out, hypoglycemia, reducing long term risks
Class 4: problem solving, heart healthy eating, healthy coping, sleep, and stress reduction, evaluating data
• Discover insights to help manage diabetes and prevent complications that can result from poorly managed diabetes.
The aim is to help put the individual living with diabetes in the driver's seat by providing lifestyle tools to support better blood sugar management.
