All Covered Persons with KPIC PPO and POS product plans, serviced by Permanente Advantage have the option to obtain services from a participating (contracted, in-network) or non-participating (non-contracted or out-of-network) provider. If you wish to find a participating (contracted or in-network) provider or facility within PHCS/Multiplan or First Health Network, please contact the appropriate network designated to the member’s region below. If using the Cigna PPO Network for care in non-Kaiser Permanente states, the Cigna PPO providers are responsible for obtaining precertification on your behalf when precertification is required. You won’t be financially responsible if a Cigna PPO provider fails to obtain precertification for covered services.

Kaiser Permanente States:

The PHCS & First Health (CO State Only) Network provides access to care in the Kaiser Permanente states of CA, CO, GA, HI, MD, VA, and DC.

Non-Kaiser Permanente States:

The Cigna PPO Network* only provides access to care in non-Kaiser Permanente states.

For services established within a Kaiser Permanente State (CA, CO, GA, HI, MD, VA, DC) using the PHCS or OOA Network Benefit:

Please contact Permanente Advantage for pre-certification questions by calling the phone # listed under Contact Us, Monday through Friday, 6 a.m. to 6 p.m. PST. If you call after hours, you may leave a message, and your call will be returned on the next business day.

For services established outside a Kaiser Permanente State using the Cigna PPO Network*:

Providers who are under the Cigna PPO Network will be responsible for obtaining pre-certification on behalf of the member when pre-certification is required.
