- Please note that Self-Funded member claims may not be available in this self-service tool. If your inquiry is about a Self-Funded member claim, please sign on to the Self-Funded plans online tool.
- You may now submit online disputes, appeals, supplemental claim attachments or request for information (RFI) via Online Affiliate. Learn more about our new Claim submission tools and partnership.
- For any changes related to your organization, group, or practice (e.g., to add, delete, or update locations, billing information, participating practitioners), please download and complete the Provider Profile Information Form (PPIF) and email to TPMG-MSC-ProvSvcs@kp.org.
