• This is a reminder that the State of Hawaii Med-Quest Division we will be conducting a survey to gather feedback on the Quest Line of Business. This is a random survey conducted by a company called "Logit”. If you are selected to participate in this survey, you will receive a letter and will have the option to either complete online or to mail your response.
  The survey will be open from 07/02/25 through 08/29/2025 and should only take a few moments to complete.
