Formulary
There are several options to access the Commercial and Medicare Part D Formularies:
Providers may access our formulary online using Lexi-Comp Online™
To obtain a login and password to Lexicomp OnlineTM or to request a paper copy of our formulary, please contact our Pharmacy Administration department at (808) 432-5549.
Providers may also access our formulary through kp.org.
Non-formulary exceptions
The prescriber makes the final decision on appropriate therapy for his/her patient. If the established Drug Formulary does not adequately meet the member’s/patient’s clinical needs, a Non-Formulary Drug may be considered for use on an exception basis. Non-formulary drugs are used only if:
- The member/patient fails to respond to the maximum doses of a formulary drug, as documented in the medical record.
- There is a high expectation that the member/patient will fail maximum doses of a formulary drug, as documented in the medical record.
- The member is allergic or intolerant to the formulary drug, as documented in the medical record.
- There is no other dosage form available on the formulary, as documented in the medical record.
Mail order service
Members may conveniently order prescription refills to fill prescriptions for maintenance drugs on the formulary list in several ways:
- Online from the Kaiser Permanente member website
- Call the automated refill line at 808-643-RxRx (808-643-7979)
- Mail order using the prescription or refill request mail order form available at any Kaiser Permanente pharmacy.
