Getting Services
 

For services within Kaiser Permanente, you do not need a referral.

For services outside of Kaiser Permanente, prior authorization is required. To start the process, talk to your Kaiser Permanente ob-gyn clinician. If you have not already been seen within Kaiser Permanente for fertility care, please contact your PCP or ob-gyn to start this process. If you have already been seen for fertility care within Kaiser Permanente, please reach out to the fertility team to assist you in next steps.

We recommend having a pelvic exam and preconception counseling before your fertility services appointment. This should be done up to 12 months before your appointment.

Initial Care and Coverage
 

At the initial appointment:

  • The fertility nurse will review pricing.
  • We will order lab or diagnostic testing.
  • We will schedule an appointment with a fertility clinician.

Cryopreservation is the process of freezing eggs, sperm, or embryos to subzero temperatures for later use.

This service is covered for preservation of material during covered treatment.

For cryopreservation benefit coverage for long-term storage view your benefit summary or call Membership Services at 1-800-813-200 for more information.

Should your Kaiser Permanente coverage end, you will be responsible for any storage costs. Contact the storage facility to discuss your options.

Services and Facilities

Kaiser Permanente facilities:

  • Intrauterine insemination, also known as artificial insemination (IUI)

Contracted participating providers with a prior authorization from a Kaiser Permanente provider:

  • Diagnostic labs
  • Diagnostic testing
  • Intrauterine insemination, also known as artificial insemination (IUI)
  • Assisted reproductive technology fertilization services. This is a process of fertilization where an egg is combined with sperm in vitro. services including: IVF, GIFT, ZIFT, cryopreservation

Types of Assisted reproductive technology fertilization services:

  • In vitro fertilization (IVF) is a process of fertilization where an egg is combined with sperm in vitro.
  • Gamete intrafallopian transfer (GIFT) is when eggs are removed from the ovaries and placed in one of the fallopian tubes, along with the sperm.
  • Zygote intrafallopian transfer (ZIFT) is similar to IVF and GIFT, but for this procedure, eggs are fertilized in the lab before being placed in the fallopian tubes.
  • Cryopreservation is the process of freezing eggs, sperm, or embryos to subzero temperatures for later use.
     

Members can visit any Kaiser Permanente lab for most fertility lab services, with the exception of semen analysis. Semen analysis is limited to the following locations:

Hours are Monday through Friday, 9 a.m. to 4 p.m., visit kp.org/locations for more information. 

Semen drop-offs on weekends or after 4 p.m. on weekdays are not recommended.

Note: Kaiser Permanente does not contract with Reproductive Medicine Laboratory (RML) for lab services. Members who use this lab but will be required to pay for services and submit for reimbursement for coverable services.

What is a Hysterosalpingogram?

Hysterosalpingogram is an X-ray to outline the internal shape of the uterus and show whether the fallopian tubes are blocked.

What is a Sonohysterogram?

A type of ultrasound of the uterus. It also is called saline infusion sonography (SIS).

Note: Prior authorization is required for services with contracted facilities. Refer to your Evidence of Coverage for covered services or contact Member Services at 1-800-813-2000 (TTY 711).

Note: After the external lab order has been received, Client Services will reach out to the member to choose a Kaiser Permanente lab for collection. Client Services will fax a copy of the order to the lab the member has chosen.


Treatment and Pricing
 
The Preregistration Department will provide cost estimates for services provided within Kaiser Permanente. For the cost of services provided outside of Kaiser Permanente, please contact your treating provider. All ART services are provided by prior authorization and referral for those who have a covered benefit.
If you are registered on kp.org and Preregistration has already provided a cost estimate for fertility services, the estimate is available in your kp.org Message Center inbox.
Fertility treatments are not eligible for the Medical Financial Assistance program.

Cost & Coverage
 

Yes, you can request tax statements from Member Services for medical services, dental services, and prescriptions. However, fertility services cannot be carved out in a separate statement. You can also view your medical payment history on kp.org (see steps below).

Note: Prescription and premium payment history is not currently available online.
