  • Complete your health screening with Kaiser Permanente. Schedule a health screening with your Kaiser Permanente doctor if needed to get your biometric screenings - BMI, blood pressure, and cholesterol. 
  • Visit Bank of America’s My Wellness Page to submit your health questionnaire to keep a $250 credit.
  • Attest on Bank of America’s My Wellness Page  that you have a primary care provider (PCP) and have had an annual physical with your PCP within the last 12 months to keep an additional $250 credit.

General information

You have the opportunity to keep a wellness credit of up to $500 toward your annual medical plan contribution by completing and submitting all three voluntary 2026 U.S. Wellness Activities by the Feb. 28, 2026, deadline.

Complete your health screenings through Kaiser Permanente and visit Bank of America’s  My Wellness Page to submit your health questionnaire to keep a $250 credit.

Attest on Bank of America’s  My Wellness Page that you have a primary care provider (PCP) and have had an annual physical with your PCP within the last 12 months to keep an additional $250 credit.

Although you can complete your health screening activity through Kaiser Permanente, you will need to complete your health questionnaire on the Bank of America’s Wellness Page and attest to your annual physical visit.

Your covered spouse or partner can keep an additional $250 wellness credit by completing and submitting their health questionnaire and health screening on My Wellness, by Feb. 28, 2026. If they also attest to having a primary care provider (PCP) and have had an annual physical with their PCP within the last 12 months, they’ll keep an additional $250 credit toward your annual medical plan contribution.

Note: You can complete each of these activities in the order that makes sense for you. However, to keep the additional $250 credit for attesting to having a PCP and having had an annual physical within the past 12 months, you must also complete the health questionnaire and health screening.

Bank of America subscribers and their spouse or partner, 18 years of age and older, who are enrolled in the Kaiser Permanente health plan through Bank of America can participate in the program through by visiting Bank of America’s My Wellness Page . Adult dependent children are not eligible for the wellness credit. Ask your program administrator for more details regarding adult dependent children.

The Wellness program runs from Oct. 1, 2025, through Feb. 28, 2026. This means you need to complete and submit all three voluntary wellness activities during these dates in order to keep your Wellness credit. However, if your benefits effective date is after Jan. 1, 2026, contact Well Member Services at 1-844-939-5100, or bofa.mywellness@support.well.co for more details.

When you finish your health screening at Kaiser Permanente, you don’t have to report that you completed it. The health screening will automatically be reported to your Wellness Program administrator.

Note: It may take up to 4 weeks for certain activities to appear as complete.

If your mailing address is wrong, contact the GHR Service Center by phone, 1-800.556.6044, Monday through Friday from 8 a.m. – 8 p.m. Eastern.

If your email address is wrong, go to kp.org to update it:

  • Log in with your kp.org user ID and password.
  • Click on your name on the top right and select "Profile and Preferences."
  • Click "View all of your personal information" under "Personal information."
  • Click "Email address" and then "Change your email address."
  • Enter your updated email address and click "Save."

Note: It can take up to four weeks to update the address in our system.

Kaiser Permanente will track whether you complete the health screening activity and will notify Bank of America when you have completed the health screening activity at a Kaiser Permanente facility and your health screening results. This allows you to keep your credit toward your annual medical plan contribution.

To find out how to get the screenings you need at a Kaiser Permanente facility, please call your doctor's office or the customer service number on the back of your Kaiser Permanente ID card.

Yes. If a current health condition prevents you from taking any or all screenings, or if a screening is no longer required for your age and gender, please contact Well Member Services at 1-844-939-5100, or bofa.mywellness@support.well.co, to request a waiver for that activity.

You can contact Well Member Services at 1-844-939-5100, or bofa.mywellness@support.well.co with specific questions related to the Wellness Program. They can give you guidance on which activities you need to complete and answer reward-related questions.

Alternatively, you can contact Kaiser Permanente Rewards Customer Service at 1-866-300-9867 or email them at RewardsCustomerService@kp.org and provide the following information:

  • Your name
  • A brief statement of your question or issue
  • You are inquiring about the Bank of America program
  • Your daytime phone number

The 2026 Wellness program runs from 10/1/2025 through 2/28/2026 and is open to all Bank of America employees and their spouses/partners who are Kaiser Permanente members who are enrolled in the Kaiser Permanente health plan through Bank of America. The program is available to all eligible individuals regardless of their health status. If you think you can’t meet a requirement because of your health, please go to your My Wellness page for further information.

Workforce health programs and rewards are separate services that are not health plan benefits and may be discontinued at any time.
