Your health information is easy to access
Adult patients discharged after July 1, 2017 can access portions of their own electronic medical report through
in-person registration with our Medical Records Department located adjacent to the Emergency Department entrance at Maui Memorial Medical Center. A valid photo ID will be needed and dates of your hospitalization. Medical Records hours of operation are from 7:30 a.m. to 4 p.m. weekdays. They can be reached at

If you are a Kaiser Permanente member in the Hawaii region, you are able to access your Maui Health information through the Kaiser Permanente website at kp.org if you are an existing user or self-register for access at kp.org/registernow.
Kaiser Permanente members visiting Hawaii, will need a separate user ID and password to view information about care received from Maui Health.
For the best experience, use a computer, not a smartphone, to access your health information. If you have questions or trouble signing on, call Kaiser Permanente support at (800) 556-7677 and inform the representative that you are a patient of Maui Health.
