Patients discharged before July 1, 2017, please call the Medical Records office at (808) 442-5213.

Patients discharged after July 1, 2017, may access portions of their medical records on the Maui Health Patient Portal.

• Non-Kaiser Permanente Health Plan members: Click here to register or log in to the Maui Health Patient Portal.
  • New account registration requires an email address and patient Medical Record Number (MRN). For help locating your Maui Health MRN, please call (808) 442-5213.
• Hawaii Region* KP Health Plan members: Access Maui Health medical records in your online KP account at kp.org.
• Non-Hawaii KP Health Plan members: Please call KP at (800) 556-7677 for assistance.

For technical issues, portal login or password assistance, call Kaiser Permanente support at (800) 556-7677 and inform the representative that you are a Maui Health patient.
