Your personalized action plan

NOTE:
If you are eligible for Group retiree benefits, you typically won’t cancel your
existing coverage. Reach out to your benefits administrator to advise them of
your intent to enroll.

Avoid getting billed twice. Once your Kaiser Permanente Medicare health plan begins, you need to ask how to cancel your current coverage.

  • If you have an Exchange plan: Contact the Exchange unit you purchased your plan through for guidance.

  • If you have a Kaiser Permanente Individual and Family plan: Submit a Kaiser Permanente Individual & Family plan Disenrollment Request form, or contact Member Services at 1-800-464-4000 (TTY 711), 8 a.m. to 8 p.m., 7 days a week, for more information.
