WHEN YOU NEED TO COMPLETE THIS TASKNOTE: Plan to begin your Medicare Advantage application 3 months before your 65th birthday. This will help you avoid gaps in your future coverage.
Once you receive proof of your Medicare Parts A & B entitlements, complete the Kaiser Permanente Medicare Advantage Enrollment Form. Once enrolled, your Kaiser Permanente Medicare health plan will cover all benefits under Medicare Parts A & B and includes Part D prescription drug coverage. Your Kaiser Permanente doctor’s office and many of our services like pharmacy, lab, and X-ray are often in one location.

In California, Hawaii, and Washington, Kaiser Permanente is an HMO plan with a Medicare contract. In Colorado, Kaiser Permanente is an HMO, HMO-POS and PPO plan with Medicare contracts. In Oregon, Southwest Washington, Georgia, Maryland, Virginia and the District of Columbia, Kaiser Permanente is an HMO and HMO-POS plan with a Medicare contract. Enrollment in Kaiser Permanente depends on contract renewal.
