How can I update the information on my Kaiser Permanente plan?

Here's how to update information such as your name, address, date of birth, or telephone number.

KP Individual and Family plans not purchased from a Health Insurance Exchange

To change your address:

Send a message to Member Services with the subject “other.” Include your new address and the names of all dependents covered by your plan.

If you are not the account holder, you may only change the address used for your own medical-related information. Send a message to Member Services with the subject “other.”

To change your name, date of birth, Social Security Number, or gender:

Complete an Account Change Form (available below) and follow the instructions.

Note: Updating your address or date of birth may cause your plan rates to change.

Federal plans

Download, print, and return an Enrollment Change Form. To download a copy of the form visit the brochures and Forms page at the Kaiser Permanente Federal Employees site.

To have a paper copy of the Federal Employees Enrollment Change Form mailed to you, send a message to Member Services through the kp.org message center.

Medi-Cal plans

Medi-Cal will process any changes and then notify Kaiser Permanente of the updated information.

Medicaid

Grandfathered plans

Complete an Account Change Form (available below) and return it to Kaiser Permanente.

Note: Changing your address or date of birth may result in a change to your plan rates.

How do I add a newborn or newly adopted child to my plan?

An overview of your options:

In California, Colorado, Georgia, Maryland, Oregon, and Virginia:

A newborn is covered under the parent’s coverage for 31 days, including the date of birth. You must request to add a newborn onto the account. (See instruction below). Forms completed at the time of birth do not enroll the newborn onto the plan.

In Washington State:

A newborn is covered under the parent’s coverage for 21 days, including the date of birth. You must request to add a newborn onto the account. (See instruction below). Forms completed at the time of birth do not enroll the newborn onto the plan.

In Hawaii:

A newborn is covered under the parent’s coverage during the birth month, if you add the newborn onto the account within 31 days. (See instruction below). Forms completed at the time of birth do not enroll the newborn onto the plan.

Group plans

You must add the newborn or adopted child to the plan through the employer within 31 days of the date of birth or adoption. If you do not enroll the child through the employer within 31 days of the date of birth or adoption, you must wait until the next open enrollment period or qualifying event to add the dependent to the group plan.

For more information about how to enroll your dependent, please contact your employer’s Human Resources department. Once you have completed the employer’s enrollment process, the employer will submit the requested changes to Kaiser Permanente.

KP Individual and Family plans not purchased from a Health Insurance Exchange

(CA, CO, HI, Mid-Atlantic States, and Northwest regions)

You may add the new dependent by completing and return an Account Change Form (available below)within 60 days of the birth or adoption. If you do not add the child within 60 days of the birth or adoption, you must wait until the next open enrollment period or qualifying event to add the dependent to his or her plan.

Please include supporting documentation for adoptions and births not a Kaiser Permanente facility.

KP Individual and Family Plans purchased from a Health Insurance Exchange

(CA, CO, Mid-Atlantic States, and Northwest regions)

You must add the newborn or adopted child to the plan through the exchange within 60 days of the date of birth or adoption. If you do not enroll the child through the exchange within 60 days of the birth or adoption, you must wait until the next open enrollment period to add the dependent to his/her plan.
