Specific Terms for Pharmacy Documents

Patient Prescription Insert – these are paper handouts that come with many prescription medicines. The guides address issues that are specific to drugs and drug classes, and they contain FDA-approved information that can help patients use the medicine safely and effectively. Not all medicines have patient information and if you have questions about your medicines you should always talk with your health care provider or pharmacist. 

Specific terms of consent agreement

  1. If you do not consent to electronic delivery of your pharmacy documents, then a paper copy will be provided to you as you currently receive them.
  2. Your consent to receive pharmacy documents will be effective on an ongoing basis until you withdraw your consent.
  3. If, after providing your consent for electronic delivery you wish to obtain a paper copy of your pharmacy documents, you may request a copy from your Kaiser Permanente pharmacy.
  4. If your enrollment in your Kaiser Permanente health plan is terminated, and you continue to utilize Kaiser Permanente pharmacy services, you will revert to receiving paper pharmacy documents.  For prescriptions previously filled at a Kaiser Permanente pharmacy, you may request a copy from the Kaiser Permanente pharmacy where you obtained your prescriptions.
