About this facility

This medical office is temporary closed. Due to the COVID-19 situation, we are consolidating our medical offices to reduce unnecessary exposure for our patients and staff.

We are still here to care for you virtually or in-person at the following medical offices: Acero , Arapahoe , Aurora Centrepoint , Franklin , Lone Tree , Loveland , Parkside , Rock Creek , and Skyline . The Westminster Medical Offices are open for pharmacy services only.

Kaiser Permanente considers the following factors/criteria as it builds and maintains provider networks:

  • Size and demographics of the population to be served;
  • The inventory of provider types required to serve the population and using practitioner to member ratios and geographic access standards metrics to ensure adequacy;
  • Application of rigorous credentialing criteria encompassing but not limited to educational training, licensing, professional experience, board certification, and professional references.

Kaiser Permanente uses the same quality, member experience, or cost-related measures to select practitioners and facilities in Marketplace Silver-tier plans as it does for all other KFHP (Kaiser Foundation Health Plan) products and lines of business. Members enrolled in KFHP Marketplace plans have access to all professional, institutional and ancillary health care providers who participate in KFHP plans' contracted provider network, in accordance with the terms of the members' KFHP plan of coverage. All Kaiser Permanente Medical Group physicians and network physicians are subject to the same quality review processes and certifications.

Kaiser Permanente uses the same geographic distribution consideration to select hospitals in Marketplace plans as it does for all other KFHP products and lines of business.

Information about a practitioner is provided to us by the practitioner or the practitioner's network, or is obtained as part of the credentialing process. An authorization or referral may be required to access some providers in this directory. If you have questions, contact Member Services.

Referrals to affiliated/contracted physicians/providers in Denver/Boulder

Affiliated/contracted physicians/providers are not part of the CPMG (Colorado Permanente Medical Group). They are physicians/providers who have a contract with the CPMG.

If your CPMG physician decides that you require covered services not available internally from CPMG, he or she will refer you to an affiliated/contracted physician/provider.

You must have a written referral to the affiliated/contracted physician/provider in order for Kaiser Permanente Colorado to cover the services. A referral is a written authorization from Kaiser Permanente for you to receive a covered service from an affiliated/contracted physician/provider.

FROM I-70 EASTBOUND. Continue on I-70 E to Denver. Continue on Lincoln St. Take E 17th Ave to N Ogden St.

FROM I-70 WESTBOUND. Take exit 276B for CO-2/Colorado Blvd toward US-6 E/US-85 N. Use any lane to turn sharply left onto CO-2 S/Colorado Blvd. Turn right onto E 23rd Ave. Turn left onto N Downing St. Turn right onto E 19th Ave.

FROM US-36 WESTBOUND. Follow US-36 W and I-70 W to CO-2 S/Colorado Blvd in Denver. Take exit 276B from I-70 W. Follow CO-2 S/Colorado Blvd and E 23rd Ave to E 19th Ave.

FROM CO-86 WESTBOUND. Get on I-25 N. Follow I-25 N to S Lincoln St in Denver. Take exit 207A from I-25 N. Continue on S Lincoln St. Drive to N Ogden St.
