Patients discharged after July 1, 2017 : You can access portions of your own electronic medical report online through the Maui Health patient portal. Sign in or register below.

Alternatively, you can register in-person by visiting our Medical Records Department located adjacent to the Emergency Department entrance at Maui Memorial Medical Center. A valid photo ID will be needed and dates of your hospitalization. Medical Records hours of operation are from 7:30 a.m. to 4 p.m. weekdays. They can be reached at (808) 442-5213, TTY 711.

Patients discharged before July 1, 2017, please call the Medical Records office at (808) 442-5213, TTY 711.

If you are a Kaiser Permanente member in the Hawaii region, you are able to access your Maui Health information through the Kaiser Permanente website at kp.org if you are an existing user or self-register for access at kp.org/registernow.

Kaiser Permanente members visiting Hawaii, will need a separate user ID and password to view information about care received from Maui Health.

If you have questions or trouble signing on, call Kaiser Permanente support at (800) 556-7677, TTY 711 and inform the representative that you are a patient of Maui Health.
